✍️List of documents for submitting an application for a preferential long-term loan for IDPs - residents of the Siverskodonetsk urban territorial community of Luhansk region, who have reached the age of 60 and over

1. Statement of intent to obtain a loan with a questionnaire - filled out by the applicant.
2. The application - consent to the processing of personal data - is filled out by all adult family members.
3. A copy of the applicant's passport of a citizen of Ukraine and the passports of citizens of Ukraine of his family members (birth certificates), all completed pages of the passport-booklet, passport ID-card must be provided with a certificate-appendix or an extract from the register of the territorial community.
4. A copy of the taxpayer registration card number of the applicant and his/her family members.
5. Copy of the certificate of registration of the applicant and his family members as an internally displaced person.
6. Copies of documents confirming the degree of family relationship (marriage certificate).

All copies of documents must be duly certified (with the mark "according to the original", date, signature, surname and initials on each sheet).
